Our goal is to keep my 84 year old father, confined to a wheelchair, at home for as long as possible. Debra has been a godsend! She has helped us navigate the complexities of our Medicaid program and finding the right care aide for our specialized needs. She has helped me navigate the Community Care Medicaid program that my mom recently qualified for. Finding trusted and competent home care staff is difficult and they have been persistent and diligent about getting mom good help that fits our family. I've enjoyed working with Debra. The in- home aides have been very attentive to my wife's needs, they interact with her (rather than just sit there), keep her busy and they help me by relieving me of some of my household chores. Ida was my husband’s caretaker and our whole family came to love her and treat her like part of our family. She cared about my husband and was respectful and kind towards him. She started part time and then came 5 days a week. She did much more than was expected and therefore it helped me when I was home to focus on him more than house hold chores. She was always prompt and dependable which was important to me. I could give suggestions and she would accommodate my requests.

Together, we assess and understand your loved one’s unique situation and recommend a personalized plan for their particular needs. This gets them the care they need as soon as they need it.

1 The Initial Call and Meeting

From the moment you call our office, we begin considering you and your loved one’s specific needs. On the initial call, we ask questions to form a basic needs assessment. After the call, we set up an in-home visit with you and the rest of your family. This visit allows us to gather even more information, answer questions, and let you understand how we can help.

The Personalized Care Plan

With Elite Home Care, you will always know what services are being provided to your loved ones. After our initial meeting, we develop a Personalized Care Plan. This plan is tailored to your loved one’s specific needs. We make detailed recommendations on exactly what kind of services we feel would benefit them. We then arrange a follow-up meeting with you to go over the Personalized Care Plan and modify it as you see fit.

3 Caregiver Matching

Once you approve the Personalized Care Plan, a Personal Care Coordinator will match the most appropriate caregiver based on numerous factors: services needed, your loved one’s interests, preferences, and the compatibility of personalities between the caregiver and your loved one. By looking at the whole situation, we see to it that your loved ones are matched with the best caregivers as possible. Of course, if you are not satisfied with the referred caregiver, we are happy to select a different caregiver to meet your needs.

4 Care Supervision

Our caregivers keep extensive notes about the care they provide. Those notes are kept at your loved one’s home and are available at any time
